i try to install last autopatcher on vista sp1 unsuccessfuly
after some research i find the problem is related to new Windows Update v7 (WindowsUpdateAgent30-x86.exe)
you have to install is first and then reboot before install others updates
if you don't do this on the 3rd cycle of vista updates they undo all the updates
this works for me so if you have the same problem give it a try
